Case Management Southern Tier Environments for Living provides case management services in Cattaraugus County. This service provides goal-oriented, systematic support for individuals served through advocacy, ongoing coordination and linkage among other core program areas and other community resources. This program serves 12 individuals through the Intensive Case Management (ICM) and 20 individuals through the Supportive Case Management (SCM) programs. The targeted population is persons with severe and persistent mental illness and mentally ill chemical abusers who reside in Cattaraugus County. Each participant in the program is visited in accordance with the assigned level of case management service. SCM individuals receive two visits per month and ICM individuals four visits a month. Visits are approximately 30 minutes in duration. Program hours are from 9 to 5 p.m. with 24 hour on call coverage for crisis situations. Services
